PowerSchool

(1)    Returning Students (Required): Update demographic information in PowerSchool

  • Login to PowerSchool, navigate to the bottom of the column on the left, and click on PowerSchool Registration.

  • Complete the form that is available to you. This will be either the New Student Enrollment Form (if you did not complete this step last year) or the Annual Student Registration Form (if you did complete last year's form).

(2)   New Students: Follow the instructions that will be sent to your email address on file (subject line, Princeton Charter School Parent Portal Access) on Friday, 8/16/19. This will explain how to create your PowerSchool Account. If you have not received this email by end of day on 8/16/19, please contact the technology department at techsupport@princetoncharter.org
 

Permission Forms

(1)  Complete the Google Form that will be sent via email on 8/2/19.

If you do not receive the Google form:

  • Please check your email and other contact information in PowerSchool. The school uses the Guardian Email field for communications.

  • Only if you are unable to access the Google form, download and return these forms no later than 8/30/19.

ASP – After School Program

(1)   Download, complete, and return all forms that apply to your child before the start of school, or prior to the first day on which your child will attend.

(1)    Please submit forms as soon as possible to allow for processing and implementation.

Medical Forms

If you have any questions, please contact the nurse at nurse@princetoncharter.org

All forms must be completed and returned no later than Friday, August 30, 2019. Staff will be on hand to receive paperwork starting Monday, August 26, 2019.  If you mail the forms, please make sure that they reach the school by the August 30, 2018 deadline.

If you hand deliver the forms, per HIPAA privacy regulations, submit Health Forms in a sealed envelope. These must be handed to the School Nurse (K-4 building.)

(1)   Required Medical Forms – All students

  • Student Health Questionnaire & Consent for the Release of Medical Information. Click here.

  • Authorization to administer non-prescribed and over-the-counter medications. Click here.

  • Consent for Emergency Medical Treatment & Health Insurance information. Click here.

(2)  Required for Grades K and 3

Kindergarten, 1st, 2nd, 3rd, 4th and 5th Grade Physical Evaluation Form. Click here.

(3)  Other Medical Forms as Needed: Download, complete, and return all forms that apply to your child.
